Gaps in the care of diabetes and cardiovascular disease continue to exist despite clear evidence and clinical guidelines that describe best practices. The COORDINATE-Diabetes study was designed to address these gaps through a targeted intervention to educate healthcare providers and remove barriers to providing evidence-based care. These publications describe the rationale for COORDINATE-Diabetes and what we learned from this important study.
